Listed below are the links for the housing and hosting forms for this June. Please note that there are two forms - one for non-FVL students who are staying at a host home, and one form for FVL families who will be hosting students. Be sure to use the correct form.
In the past, some of our LVW members from outside the Appleton area have stayed with relatives or friends of the family who live in the area. If this is the case, please forward this email to your friends/relatives so they can use the online sign up for hosting students. If you are a non-FVL family but WILL be hosting students (for example - friends or relatives of LVW members), please use the LVW 2015 Hosting Form for FVL Students.
FVL families should submit the form EVEN IF YOU ARE NOT ABLE TO HOST. There is a place on the form to indicate whether you can or can not host students.
Please submit the form by Friday, May 15. Parents and students should work together to fill out the form, because there is information that both will need to fill out such as medical information, emergency contact information, and listing students who are planning to stay with each other or hosting specific students.
LVW students will be staying in host homes on the following nights: June 9, 10, 11; 16, 17, 18; 24.
